As we continue to look for ways to improve our referral and staffing search process, we are asking all providers to complete the form linked here: PROVIDER COVERAGE AREA and email it to me at wlittlefield@seniorsplus.org. Many Maine counties are very large. This form will help our Care Coordinators determine which parts of each county our providers cover.
